Too big width -
If the bra has a width that is too big, can you replace it with an underwire with smaller width? Would that work?

Yes and no. If it's not the same length as the previous wire it will slide around in the channel. It might also just bend to the width of the original channel unless you use very hard wires, like Panache. Changing the wire also changes the depth of the bra so just be aware of that.
The sliding around with a wire that is shorter than the channel can be addressed with a few stitches to lock the wire in place. Place the stitches at both ends of the channel where the wire ends. There is empty space but it is not impinging on sensitive areas.
